To protect the health and safety of the Hanford community, and in line with recommendations from government and public health authorities, Hanford respectfully requires all personnel including all outside contractors to complete the following questionnaire prior to being granted access to the company.

This form should be completed and submitted no more than 2 hours prior to your arrival at Hanford.
